Pay Range: Estimated $50.00–$75.00 per hour or $95,000–$130,000 annually, based on license, experience, schedule, and assignment type

Position Summary
KARE Masters Home Health is seeking qualified Nurse Practitioners to support patient care, assessment, care coordination, and health management for individuals receiving home health services. This role may involve working with patients who are recovering from illness, surgery, disability, or chronic health conditions.
The Nurse Practitioner may support clinical evaluations, treatment recommendations, chronic disease management, patient education, provider communication, and care planning in collaboration with the home health care team.
Key Responsibilities
- Assess patient health needs and support care planning.
- Collaborate with physicians, nurses, therapists, aides, patients, and families.
- Support chronic disease management and patient education.
- Assist with care coordination for patients with complex medical needs.
- Review patient progress, care concerns, and treatment needs.
- Provide guidance related to safety, medication management, rehabilitation needs, and ongoing care.
- Help reduce rehospitalization risk through proactive care planning and patient support.
- Document services and recommendations according to agency and clinical standards.
- Communicate professionally with patients, caregivers, and healthcare providers.
- Follow all state, federal, agency, and confidentiality requirements.
Qualifications
- Active Michigan Nurse Practitioner license required.
- Master’s degree or higher in nursing required.
- Home health, primary care, geriatric care, chronic disease management, rehabilitation, or care management experience preferred.
- Strong assessment, communication, documentation, and care coordination skills required.
- Ability to work independently and collaboratively with a care team.
- Reliable transportation may be required for home-based visits.
- Must be able to complete required onboarding, credentialing, background checks, and employment screenings.
