This RN works on an as-needed basis visiting patients in their homes, assessing their health, providing nursing care, and helping them stay safe and independent.
What This Work Looks Like Day-to-Day

Review your patient assignment for the day based on PRN scheduling needs

Drive to patients’ homes to assess their condition and provide direct nursing care

Perform wound care, medication management, IV support, and other physician-ordered interventions

Talk with patients and families about their health goals and how to manage conditions at home

Update care plans based on changes in patient condition

Coordinate with doctors, case managers, and other team members to ensure patients receive complete and timely care

Document all visits and assessments before closing out your shift

Provide health education, coaching, and motivation to help patients stay independent

Support home health aides and LPNs when needed

End your day by completing charts, documentation, and follow-up communication

Job Description
Trinity Health At Home is looking for a compassionate PRN Homecare RN to deliver high-quality nursing care to patients in their homes. This nurse will conduct assessments, implement individualized care plans, monitor progress, and educate patients and families to promote independence and improved health outcomes. The RN will support care coordination and partner with physicians and case managers to ensure exceptional service delivery.
Education & Requirements

Graduate of an accredited nursing program; BSN preferred

Current Michigan RN license
-
Minimum 1 year professional RN experience required

Home health experience preferred

Strong computer and technology skills

Valid driver’s license and reliable transportation required
