Do you have a passion for supporting youth and families in crisis? As a Case Manager, you'll play an integral role in ensuring the safe reunification of children and youth with responsible sponsors. In this role, you will work in a shelter environment assisting youth in the reunification process by identifying qualified sponsors and adhering to stringent state and federal guidelines. Wage:
$27.54/hr
Mandatory: MUST be a Spanish Speaker
Key Responsibilities:
- Initial Assessments: Conduct initial assessments for children and youth upon arrival at the shelter. Interview the youth to identify opportunities for reunification with a qualified sponsor and gather necessary historical information for their safety.
- Sponsor Vetting: Regularly interview and assess potential sponsors to ensure the legitimacy of relationships with the youth. Conduct virtual walkthroughs, proof of address checks, and reference checks to confirm qualifications.
- Documentation & Reporting: Complete required assessments and input all information into the UC Portal and internal systems. Ensure that all documentation meets state and federal standards for reunification.
- Reunification Process: Coordinate and initiate the reunification process once a qualified sponsor is identified, ensuring timelines and all necessary approvals are met.
- Service Planning & Coordination: Develop and maintain Individual Service Plans (ISPs) for each youth, communicating any changes or updates. Attend and coordinate Treatment Team meetings and participate in weekly staffing.
- Safety & Compliance: Enforce the Zero Tolerance Policy regarding abuse and neglect. Follow state and federal regulations, including child protective services guidelines and HIPAA confidentiality standards.
- Case Management: Ensure timely submission of case logs and provide any necessary follow-up on Significant Incident Reports (SIRs) or other incidents that require attention.
- Collaboration: Work closely with the Social Services Supervisor, consulates, legal services, and other departments to facilitate the reunification process and ensure the youth’s well-being.
- Training & Development: Complete required training in Managing Aggressive Behavior (NRCYS), CPR/First Aid, and other relevant certifications.

Qualifications:
Education Requirements:
- Bachelor’s degree in a human behavioral science or related field (e.g., Social Work, Psychology, Sociology).
- At least 25% of credits must be in human behavioral sciences if you have a different Bachelor's degree.
- MUST speak Spanish
Experience Requirements:
- 1 year of experience working with youth, preferably in a child-care institution or related field. Internship or volunteer experience is also valuable.
- Experience in case management and working with state and federal policies, particularly ORR and MDHHS regulations, is beneficial.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint) and case management software.
- Experience using a tablet or laptop for data entry and communication.
Additional Skills and Abilities:
- Language Proficiency: Fluency in English and Spanish (both written and verbal) is required; additional language skills are a plus.
- Strong Communication Skills: Ability to communicate effectively with children, families, colleagues, and external stakeholders. Ability to write reports and present information clearly.
- Attention to Detail: Strong organizational skills and the ability to maintain thorough, accurate documentation.
- Cultural Competence: Ability to interact with children and families from diverse cultural backgrounds in a respectful and responsive manner.
- Critical Thinking: Ability to solve problems effectively and proactively seek guidance when needed.

Standard Requirements:
- Age Requirement: Must be 21 years of age or older to work directly with youth.
- Background Checks: Must pass criminal background checks (including national fingerprinting), CPS checks, drug testing, and TB screening.
- Immunization: Proof of immunizations (varicella, measles, mumps, rubella, tetanus, Tdap, flu) will be required.
- Certifications: Must obtain Managing Aggressive Behavior (NRCYS) and CPR/First Aid certification.
- Physical Demands: Ability to stand, walk, stoop, squat, kneel, and lift up to 25 lbs. for extended periods. Work will also involve sitting for long periods to enter documentation.
