Eisenhower Center is growing and seeking compassionate, dependable Direct Service Professionals to support residents in achieving their personal goals and greater independence.

Key Responsibilities
- Provide daily support and meaningful opportunities that help residents achieve their individual goals.
- Follow each resident's person-centered and behavioral support plans.
- Provide compassionate, respectful, and high-quality care while maintaining a safe and healthy environment.
- Build trusting relationships while protecting residents' rights, dignity, and confidentiality.
- Assist residents with activities of daily living (ADLs) and other support needs.
- Transport residents to appointments and community activities as needed.
- Attend appointments, advocate for residents, and obtain necessary documentation when required.
- Remain awake and immediately accessible when assigned to homes requiring 24/7 care.
- Complete Daily Progress Notes (DPN), Direct Data Sheets (DDS), and other required documentation accurately and on time.
- Communicate changes in resident behavior, needs, or safety concerns to appropriate staff.
- Follow all organizational policies, procedures, training requirements, and safety standards.
- Maintain a positive and professional attitude with residents, coworkers, and visitors.
- Arrive on time for scheduled shifts and participate in shift reports during staff transitions.
- Work assigned schedules consistently. Additional shifts may be available with approval.
- Perform other duties as assigned.

- Ability to communicate effectively with residents, families, and coworkers.
Qualifications
- High school diploma or GED.
- Valid driver's license and reliable transportation.
- Previous healthcare or caregiving experience preferred.
- Experience providing direct care to individuals with disabilities preferred.
- Ability to pass all required background screenings.
- Ability to follow instructions, policies, and procedures.
- Ability to meet the physical and emotional requirements of the position.
