Position Summary
The Senior Alliance is seeking a compassionate Registered Nurse (RN) Care Manager to conduct comprehensive health assessments for older adults and adults with disabilities receiving community-based services. The RN partners with a Social Worker to develop, coordinate, and monitor individualized care plans that help participants remain safely in their homes.
Responsibilities

Conduct comprehensive nursing assessments and reassessments.

Evaluate participants' medical, functional, and healthcare needs.

Develop individualized care plans with the Social Worker.

Coordinate services with physicians, hospitals, home health agencies, and community providers.

Monitor participant progress and update care plans as needed.

Work with family members and caregivers to strengthen support systems.

Maintain accurate documentation and electronic participant records.

Ensure compliance with program standards and funding requirements.

Assist with reports and documentation for Michigan Aging & Adult Services, MDHHS, and other funding agencies.

Perform additional duties as assigned.
Qualifications

Current Michigan Registered Nurse (RN) license required.

Bachelor of Science in Nursing (BSN) preferred.

Minimum of two years of nursing experience in community health, aging services, home care, human services, or a nonprofit setting.

Experience with older adults preferred.

Knowledge of community resources and the aging network preferred.

Experience with electronic documentation and third-party reimbursement systems preferred.

Valid Michigan driver's license and reliable transportation required.
