A Wraparound Coordinator, under general supervision, develops family-centered service plans for seriously emotionally disturbed (SED) children and their families. This employee will identify, coordinate, and link services with various community agencies to develop a community team to provide services to SED children. A Wraparound Coordinator will facilitate family-centered meetings with SED children and their families to gather information that will aid in the wraparound process. The Wraparound Coordinator will advocate for SED children and their families with schools, court systems, and social service agencies. An employee in this class will manage and monitor individual wraparound budget plans, provide data for outcome review, and perform other work duties as assigned.
