The COFR Case Manager, under general direction and supervision of the COFR Specialist, provides comprehensive community services for out-of-county individuals with mental illness, intellectual/developmental disabilities, youth, and autism disorder. They will have knowledge and understanding of specialized Adult Foster Care (AFC) homes while providing assessments, treatment planning, coordinating, monitoring, review, and advocacy for out-of-county consumers. They will also provide supportive services, crisis intervention, and mental health-related education for consumers, families, guardians/ payees, and significant others while completing, coordinating, obtaining and monitoring documentation of services provided.
