Formal education required:
- Masters Degree from an accredited Nurse Practitioner Program
- Completion of a Nurse Practitioner program with certification
Experience/skills/ability required (number of years and type):
- 3-years experience as a Certified Nurse Practitioner
- Knowledge of and ability to apply clinical science to practice
- Expertise in implementing the clinical process to meet patient care needs
- Knowledge and skills in implementing complex clinical procedures under supervision
- Analytical ability to identify clinical problems, formulate and implement a plan of action
- Knowledge of adult learning principles and skills in conducting educational sessions
- Flexibility to work at assigned Medical Centers

- 5-years experience as a Certified Nurse Practitioner

- Experience in childbirth education and lactation consultation

Specialized training and/or certification/license:
- Valid State of Michigan Registered Nurse License
- Nurse Practitioner Certification
- Current BCLS or CPR Certification and ACLS Certification
- Valid State of Michigan Driver's License
PRIMARY RESPONSIBILITIES:
- Direct, coordinate and participate in the care of a designated group of patients; manage patient care problems according to pre-established protocols.
- Obtain and document patient's health history; perform and document physical examinations and write prescriptions.
- Provide preventative health measures within prescribed guidelines and instructions of Medical Director or assigned physician.
- Order and interpret diagnostic tests, i.e., laboratory work, x-rays, EKG.
- Provide leadership in the development and implementation of case management protocols of high risk/high volume clients to achieve cost effective and quality care objectives.
- Review caseload with physician colleague on a regular basis to assure collaboration and adherence or protocols.
- Order, evaluate and follow-up diagnostic tests according to established protocols.
- Documentation meets all standards and policies including all findings, interventions and results.
- Instruct and counsel individuals and/or families or groups about health or defined health problems; participates in formal patient education programs and design, implement and evaluate patient health teaching education plans.
- Administer medications, treatments and immunizations as necessary according to agency policies and procedures.
- Manage and establish treatment plans for benign gynecological diseases and low risk obstetrical patients.
- Utilize community resources in planning appropriate patient care and act as a liaison by relaying information necessary for continuity of care to other health professionals and agencies.
- Coordinate services between disciplines to ensure continuity of patient care.
- Participate in continuing education to enhance own professional growth in clinical practice and teaching methodologies.
- Responsible for diagnosing and treating common acute illnesses; and treating chronic illnesses such as diabetes and hypertension.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Provides patient and/or family with health education, counseling and referrals to other healthcare professionals and community resources when appropriate.
- Treats staff, patients and families with respect and dignity.
- Collaborates with physicians and other members of the health team to plan optimal care for the patient.
- Works with the patient and family to formulate a patient treatment and education plan.
- Provides follow-up care, determines effectiveness of the treatment plan, reassesses and changes the plan as needed.
- Participates in clinical management meetings; attends all organizational and medical staff committee meetings as appropriate.
- Administers and adheres to corporate and departmental policies, practices and procedures.
