Licensed Practical Nurse (LPN) – Bronson at Home – Full-Time Days
Location: Bronson at Home Schedule: First Shift, Full Time (40 hours/week)
Overview
Bronson at Home is seeking a dedicated Licensed Practical Nurse to support patient care in a collaborative, team-focused environment. Under provider direction, the LPN delivers essential clinical support, assists with patient assessments, and ensures accurate documentation to promote safe, high-quality care.
What You’ll Do

Record vital signs, immunizations, and complete initial patient assessments.

Obtain concise patient histories and provide clinical information to providers.

Document patient care, test results, and chart updates accurately.

Educate patients and families on medications, treatments, and referrals.

Prepare daily patient charts and ensure documentation is complete before appointments.

Assist with minor and emergency procedures, offering reassurance and support to patients.

Triage patient calls to determine urgency and appropriate next steps.

Schedule tests and procedures and provide patients with preparation instructions.

Perform basic laboratory tests as directed.

Coordinate admissions, referrals, and prescription orders.

Dispense medications per provider orders (oral, IM, or SQ).

Maintain communication with referring providers and forward records as needed.

Support completion of required forms (Disability, Workers’ Compensation, etc.).

Participate in performance improvement activities and staff meetings.
-
Uphold confidentiality, customer service standards, and teamwork expectations.
Qualifications

High school diploma or GED required.
-
Completion of an accredited Licensed Practical Nursing program.

6–12 months of healthcare experience required.

Active LPN license in good standing with the State of Michigan.

BLS certification required.
