The Care Manager provides services to individuals with severe and persistent mental illness, serious emotional disturbance, and/or intellectual or developmental disabilities. This role ensures individuals receive the necessary supports to achieve community inclusion, independence, and meaningful participation in home and community-based settings. Responsibilities include advocacy, service coordination, linkage to supports, and ongoing monitoring to ensure quality, continuity, and accessibility of services. Core functions include service planning, assessment, coordination, and documentation.
