JOB TITLE: PT SOCIAL WORKER (TEMPORARY)

This is a temporary part-time position, up to two years, variable hours up to 29 per week, grant-funded position with limited benefits as provided for through the Earned Sick Time Act (ESTA)
GENERAL SUMMARY: This position receives general supervision from the Department Director. The person hired for this position will work closely with Bay County Dept. on Aging, Probate court staff, potential petitioners, and individuals at risk of adult guardianship to ensure meaningful access to less restrictive alternatives whenever possible.
ESSENTIAL FUNCTIONS:
1. Individual will be assigned to provide Guardianship Diversion services to seniors residing in Bay County. Referrals will be accepted and sought from the Probate Court, potential Petitioners, individuals at risk of adult guardianship, and other human service agencies.
2. Home visits will be completed for the purpose of establishing a need where guardianship is involved.
3. Individual will maintain accurate, up to date case files, including progress notes regarding actions taken on behalf of the client, and follow-up contacts. Information is entered into laptop computers. Computer familiarity and data entry skills are essential.
4. Individual will be knowledgeable about eligibility requirements for various programs, including benefits available through agencies such as the Department on Health and Human Services, Social Security Administration, and other key collaborative partners, etc. The social worker will assist frail seniors in completing application materials if assistance is not readily available through the helping agency.
5. Case Manager will educate clients about programs available to them to help maintain independence with community programs and encourage seniors to utilize these services when needed and readily available.
6. Social worker may make presentations to explain services to other community partners/agencies.
7. Individual will periodically attend training(s) to remain current about services available in the community and new developments in their relevant field.
8. Individual will develop a working relationship with representatives of other referral agencies.
9. Individual will complete outreach to home and community based service providers.
10. Consistent and punctual attendance is an essential function of this position.
11. Individual will complete other duties related to the grant as assigned when needed.

QUALIFICATIONS: Bachelor of Social Work or college student in their final year of study in the BSW program required. Reliable transportation or access to it. Must be especially sensitive to meeting the special needs of elderly clients. Applicants must have good rapport and ability to relate to senior citizens. Applicants must be able to lift and transport laptop computer from office to homes of seniors with or without reasonable accommodations. Must be proficient in use of computers. Applicants may be required to take written and/or other examinations. Applicants must have good reading, writing, and math skills necessary to complete required case file documentation, including eligibility applications, etc.
Physical Requirements (with or without reasonable accommodation): This position involves sitting most of the time and/or being mobile or standing for brief periods of time. Additional requirements include the ability to move objects according to the following weight & frequency: Generally, 10 pounds of force up to one third of the time.
