The Intake Team Leader provides the initial support and advocacy to mentally ill or substance abuse recovery adult consumers and their families. This professional collaborates with psychiatric and allied team members in assessment, diagnosing, treatment planning and intervention. This professional also provides triage for client crises.
ESSENTIAL DUTIES and/or RESPONSIBILITIES include, but may not be limited to the following:
- Reports to work on time and at the assigned location.
- Completes all required trainings within established timeframes.
- Completes all other duties as assigned.
Screens intake and referral calls.
- Provides useful and practical information to referral calls and/or clients to appropriate services.
- Handles crisis and referral calls in a professional and appropriate manner.
- Demonstrates a strong knowledge of community resources.
- Refers individuals who do not meet agency intake criteria to appropriate resources.
- Assures that clients receive services in a timely manner.
Arranges appropriate intake procedure.
- Demonstrates ability to assess clients’ needs appropriately.
- Demonstrates a good understanding of agency services.
- Assures appropriate treatment arrangements are in place for clients.
Meets with clients as necessary and provides triage for crisis clients.
- Demonstrates appropriate crisis intervention skills.
- Completes accurate and appropriate written assessments.
- Provides treatment and appropriate follow-up referrals for crisis clients.
- Progress notes accurately reflect interventions and reflect clients’ goals.
- Service plans and reviews are written in a timely manner and reflect clear goals, objectives, and interventions.
- Service plans demonstrate client needs and input
- Meets a minimum of 30 monthly direct service hours.
Assures the clients receive appropriate levels of care through providing and monitoring referrals.
- Completes intake assessments/process and ensures appropriate level of care referral.
- Participates in the plan of service meetings and provides appropriate levels of care.
- Provides linkages between programs to facilitate transfers.
- Practices Person Centered Planning principles.
- Assist and arrange for appropriate levels of care as consumers clinical situations change.
The Intake Team Leader participates in committees and assumes other tasks as requested.
- Clinician participates in organizational committees as requested.
- Prepares necessary reports for utilization review.
- When appropriate, completes other assigned responsibilities in a reasonably established timeframe, within quality expectations.
The clinician demonstrates professional and ethical behavior.
- The clinician continues to demonstrate proficiency in his or her professional practice by attending regular case conferences, in-services, and workshops and by reading professional literature.
- The clinician has no substantiated recipient rights violations filed against him or her.

The Intake Team Leader understands agency’s standards for program success.
