Job Summary
Responsibility for coordinating the services provided by multiple disciplines within and outside the health system. Exercises independent judgment in the assessment, diagnosis initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates evaluation practices to measure outcomes of programs.
Essential Functions
Coordinates program planning, implementation, and evaluation in area of accountability in collaboration with members of the health care team. Identifies opportunities for philanthropic support to support program objectives. Acts as patient advocate, crossing multidisciplinary lines to meet patient needs. Communicates, collaborates, networks with and acts as a consultant to other members of the health care team within and outside Spectrum Health. Acts as a consult in area of expertise.
Assesses the patient/significant other, health care team, and community educational needs and develops, implements, evaluates and revises teaching strategies/educational programs related to area of specialty.
Develop and maintain network with internal and external partners to support programming. Develops support systems for patient population and coordinates program with other system-wide/community support programs. Participates in Hospital/Community committees as member and consultant on quality care issues.
Initiates performance improvement studies in order to evaluate and measure the outcome of services. Make recommendations to improve the quality of services provided.
Coordinates the delivery of patient care within Program services related to area of specialty.
Performs and provides designated activities related to area of specialty.
Maintains the documentation and record organizational and external agency requirements for services provided.
Oversees selected staff as assigned by manager to coordinate program services.
Qualifications
Bachelor's Degree or equivalent related field required.
Master's Degree or equivalent related field preferred.
2 years of relevant experience related experience required.
Registered Nurse (RN) - STATE_MI State of Michigan Excludes CV Administration, HM Patient Care Logistics, Health Comm - Department, GM-Community Education, Lifestyle Medicine, PD HDVCH Child Protection, PH Medical Affairs Admin, PH Pre-Service Utility Management required Or
License Practical Nursing (LPN) - STATE_MI State of Michigan required Or
Registered Dietitian (RD) - CDR Commission on Dietetic Registration required Or
Master Social Worker (MSW-Master) - STATE_MI State of Michigan required Or
Physical Therapist - STATE_MI State of Michigan required Or
Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy required Or
Speech Language Pathologist - ASHA American Speech-Language-Hearing Association required
Ideal candidate will possess BSN and RN License, Ortho experience would be a big plus
Ideal candidate will be Flexible, able to help Develop this Role, Independent, Self-Motivated, able to work well with a Team and Independently, will display a Global Thinking Mentality as scope of work will impact all Regional Locations, will possess very strong interpersonal and communication skills, will have a passion for Ortho, for Educating Ortho Patients, for Quality work, and for helping Spectrum to adhere at the highest level of Joint Center of Excellence Designation.
This position will be Monday to Friday, No Weekends, and will be 50% Remote and 50% On Site.
This person will be interacting with many stakeholders located at Butterworth Hospital, but may work, while On-Site, at various physical locations.
