Job Description
Case Manager responsibilities include:
Providing intensive care management for patients, developing individualized care plans, triaging patients by phone, conducting prior authorization calls and collaborating with an interdisciplinary team, including physicians, to provide a comprehensive approach to care.
Case Managers will also: help minimize and prevent hospital readmissions for patients of special needs, guide patients as they navigate the complex world of screenings, treatments, and recovery, follow up with patients and families to ensure their understanding of the plan of care, utilize psychosocial expertise to serve each patient's unique needs while ensuring quality and continuity of care and form strong and enduring bonds with patients, families and multidisciplinary colleagues.
Submission Requirements
Minimum 3-5 years current Case Management experience (INPATIENT OR CLINIC SETTING) - Required
Phone Triage experience - Required
Prior Authorization (Medication) experience - Required
Adequate transportation and Valid Driver License - Required
Certification in Case Management - Preferred
BSN - Preferred
Certification Requirements
BLS (AHA)
State License Requirements
Michigan
