Job Description
Nurse Case Manager is responsible for telephonically assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. In this role, you would be responsible for:
Reviewing and connecting with members about their health.
Working side by side with other health care professionals to develop a care plan.
Provide telephonic educational support to members.
3-5 years of clinical (hospital experience).
Requirements
Registered Nurse with current Michigan driver's license.
Associate Degree or nursing diploma required.
Triage, case management and/or rehabilitation experience preferred.
Requested criteria above average computer skills (Microsoft Office suite)
