POSITION SUMMARY
The Activities Coordinator will plan, implement and supervise all resident activities, which gives the resident a purpose to live and maintain an optimal level of psychosocial functioning.
ENTRY REQUIREMENTS
Minimum of two years college education preferably in a healthcare related field or equivalent certification.
Preferred Activity Coordinator certification or experience in recreational activities.
ORGANIZATION
Under the general supervision of the Long Term Care Director of Nursing.
SPECIFIC DUTIES
Supports the Mission, Vision and Values of Munson Healthcare and Paul Oliver Memorial Hospital.
Embraces and supports the Performance Improvement philosophy of Munson Healthcare.
Promotes safety for patients, residents, visitors, co-workers and self. Supports National Patient Safety goals and initiatives.
Has basic understanding of Relationship-Based Care (RBC) principles, meets expectations outlined in Commitment To My Co-workers, and supports RBC initiatives.
Uses effective customer service/interpersonal skills at all times.

Has knowledge of hospital policy and procedure manual.
Completes a resident's history and assessment upon admission
Generates Daily/Monthly Activities Calendar Supervises and coordinates all residents activity program. Seeks out new activities regularly
Supervises and is responsible for all activity Volunteers and Activity Assistants. Coordinates staff assignment and writes their schedules, monitors their documentation and job performance, and provides input to their Annual Discussions.
Attends Care Conferences. Supervise documentation of resident activity
attendance.
Implements objective care plans for each resident. Addresses all activity problems and goals. Individualizes care plans for activities per resident, updates, assesses and changes as needed. Identify residents at risk for loneliness, boredom and helplessness and aligns their plans as appropriate.
Completes a ninety (90) day summary on each resident.
Encourages and sees that each resident is given the opportunity to participate in activities. Coordinates 1:1 outings as possible for individuals who do not prefer group dynamics.
Completes the activity section on the Minimum Data Set (MDS). Completes Resident Assessment Protocols and Care Planning.
Meets outcome requirement related to the State regulations for the Activity
Generates Monthly Newsletter (Write, Print, Package and Mail out Monthly using volunteers)
Attends and coordinates Resident Council, takes notes and compiles data for the State.
Assists with monitoring Diversional Therapy budget with Director.
Performs other duties and responsibilities as assigned.
