Qualifications
* B.A/B.S degree in Human Services field (education, pyschology, social work etc).
* Two (2) years related job experience working with individuals with I/DD, their families, and/or public service agencies.
* Instead of (1) and (2), five (5) years related job experience working with individuals with I/DD, their families, and/or public service agencies. Schooling or training in a related field may apply to the five (5) years.
Essential Functions:
* Assist persons and their families with a variety of needs which could include: meeting with individuals and/or their families to facility needed services
* Knowledge in Person Center Planning (PCP) process, Individual Education Planning Meeting (IEP), attend and/or prepare parents for their children's IEP meeting.
* Provide info and assistance in acquiring public benefits and entitlements and assist where needed.
* Plan for a secure future, including assistance or connecting with legal services for letters of intent, wills and trusts and alternaties to guardianship.
* Facilitate access to Community Mental Health services and planning process.
* Possess a valid driver's license and an insured, registered vehicle; or other reliable transportation for getting to and from work and to and from appointments.
* Disseminate information and interact with the service system and community-at-large to assure the inherent rights and dignity of persons with dvelopmental disability.
* Advocates must track and record the metrics for the individuals he or she serves, for reporting purposes
* Submitting various reports including but not limited to: Social Security, Disability updates, CMH reports.
* Ability to put personal preferences aside to serve the consumer's goals and interests.
* Demonstrates empathy and knowledge of unique problems and challenges that face families and I/DD individuals.
* Use skills in verbal and written communication
* Maintain emotional maturity.
