Duties:
- Provide direct assessment and counseling services to high risk and/or prenatal clients in Maternal Infant Health program, Community Connections program, and other community members served by Agency.
- Develop individual care plans for high-risk participants.
- Provide services in office, via tele-health, and in home settings.
- Provide constructive, cooperative working relationships with clients, coworkers, peers, and supervisors.
- Complete and maintain accurate, adequate, and timely records in computer software products such as Microsoft Office, Electronic Medical Records software, and similar products.
- Utilize effective communication and conflict resolution skills to serve clientele in a tactful, objective, respectful manner while maintaining privacy and HIPAA compliance.
Requirements:
- Bachelor of Science, with preference to Masters of Science
- Fully Licensed as Social Work in State of Michigan.
- One year of providing community health, pediatric, and/or maternal/infant services preferred.
- Valid Michigan driver’s license and reliable transportation.
Salary: $24.04 per hour in accordance with current Union contract plus full benefit package. Wage increases to $25.34 after successful probation period.
