POSITION SUMMARY:
Position provides case management services and supports to an assigned caseload of adult clients receiving outpatient mental health services. Responsible for assisting clients with case management and care coordination needs that are goal-oriented and individualized. Develops an integrated treatment plan in collaboration with the client and other service providers. Links/coordinates services, referrals, reassessment, follow up, advocacy, and monitors services.
MINIMUM JOB REQUIREMENTS:
- Bachelor’s Degree in Social Work
- State of Michigan professional license (LLBSW, LBSW, LLMSW, LMSW)
- One year of case management experience working with adults with psychiatric disorders
- Bilingual proficiency in Spanish preferred
COMPENSATION:
$37,050 - $44,000 annually (Full-time/Union)
