Schedule: Monday-Friday, 8:00am-4:30pm

Position Summary
The LifeCircles clinical team delivers primary care, palliative care, and end-of-life care for patients aged 55 and older. This position is perfect for the RN who wants to be patient-centered, creative, and collaborative in their approach to care.
The successful candidate will join an experienced Interdisciplinary Team and work closely with other RNs and healthcare professionals in a well-supported, team-based environment.
Primary role responsibilities include:
- Participating in daily interdisciplinary team meetings
- Completing periodic health assessments
- Assessing acute needs and illness in the clinic, participant’s home, and facilities (ALF/SNF)
- Advanced Care Planning
- Patient and Caregiver Education

Required Qualifications
- High School Diploma/ GED
- RN-Nursing Diploma or Degree from accredited School of Nursing or University
- Current license to practice as an RN in the State of Michigan
- BLS certification
- Minimum of 1-year experience with the frail and elderly, long-term care, or hospice population preferred
