Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations. Works with multiple medication automation systems.
Basic Qualifications:
Education - Bachelor's Degree in Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
Licenses -
Pharmacist, Licensed within 90 days
Pharmacist Control Substance within 90 days
Advanced Cardiac Life Support (ACLS) certification or Pediatric Advanced Life Support (PALS) within 180 days
Preferred Qualifications:
Experience - 2 years of experience typically gained through skills/knowledge/abilities in hospital pharmacy American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice
