PositionSummary:
Case Management Nurses function as a member of the
interdisciplinary team in the provision of patient care with the underlying
objective of enhancing the quality of clinical outcomes and patient
satisfaction. Through an understanding of the patient's clinical condition,
appropriate care delivery settings, and expected length of stay, the Nurse Case
Manager advocates for the patient and family, assuring that the established
standard of care and expected outcome are achieved in a timely manner and with
the appropriate use of resources.
EssentialFunctions and Responsibilities:
Assessment:
Completes a comprehensive assessment to identifyopportunities for intervention that are appropriate and realistic for thepatient/family's psycho-social, cultural, spiritual, and physical plan ofcare.
Assessthe patient's healthcare needs and goals; specifically targeting the physical,functional, psychosocial, environmental and financial status.
Communicateswith Utilization Management and attending physician regarding appropriatenessof patient admissions, utilization issues and when documentation does notsupport continued stay.
Planning/Implementation:
Demonstrates and understanding of medical necessityand intensity of service and incorporates payer requirements into thedevelopment of a safe, effective, and timely discharge plan.
Demonstrates an understanding of the patient'sclinical condition, social, and financial resources in order to determinethe most appropriate care setting, practice standards for evaluation,treatment delivery options (Home, SAR, SNF, LTACH), and resources requiredto support safe transition of care.
Coordinatesthe progression of care to ensure that the ongoing needs of the patient andfamily are adequately addressed.
Qualifications
Required:
CurrentState of Michigan Registered Nurse license
Threeyears of prior work experience in an acute care setting serving a similarpopulation of patients or prior Case Management experience in an acute caresetting.
