Duties and Responsibilities
Advocate on behalf of residents to staff, family and organization.
Identify the social, emotional, and economic needs of residents through a continuous evaluation process.
Participate in the care plan development and implementation of each resident. Provide intake processing as needed.
Document plans, goals, and observations of each resident in electronic records.
Work with referral sources to identify potential candidates for the center. Coordinate move-ins and required documentation.
Plan and coordinate admissions. Prepare and explain patient contract, programs, and policies to responsible party during admission. Complete all paperwork with the responsible party during the admission process.
Coordinate client's room assignment and arrival with nursing and other departments. Engage with resident/responsible party upon admission. Record in the appropriate database as required.
Coordinate residents' discharge. Complete discharge instructions and refer equipment and ongoing services as needed.
Participate in monthly Quality Assurance and Performance Improvement (QAPI) meetings and initiatives.
Compile necessary reports and statistics and submit to supervisor.
Job Qualifications
Education, Training, and Licensure/Certification
Bachelor's degree in Social Work or related field.
Licensed as a Social Worker in the State of Michigan preferred.
Experience
Two years' experience in social work.
Experience with older adults and/or persons with disabilities preferred.
Knowledge, Skills and Abilities
Basic counseling, coaching, and group counseling skills.
Ability to display compassion and sensitivity with vulnerable adults who may become angry, hostile or combative. Ability to maintain a helping role and intervene with resident care when appropriate.
Ability to communicate effectively with individuals with varying levels of cognitive and physical impairments.
Ability and willingness to work constructively with individuals from diverse cultural backgrounds.
Ability to apply principles of rational systems to solve practical problems and deal with a variety of concrete variables.
Knowledge of and ability to employ social brokering, advocating and mobilizing individuals and groups.
Basic knowledge of state and local human service systems and resources, particularly as they apply to geriatrics.
Ability to work independently and cooperatively. Ability to apply practical mathematical applications.
Excellent oral and written communications skills with the ability to read, write and speak the fluent English.
Ability to prepare business letters, expositions, summaries, and reports, using prescribed format and conforming to all rules of punctuation, grammar, diction and style.
Knowledge of various computer software applications.
