Primary Responsibilities:
- Provide case coordination activities necessary to support the Offender Success program in order to achieve stable housing, navigate community resources and assess needs to address barriers.
Job Functions:
- Responsible for the development of a written housing plan based on the input of the person being served. Measurable goals and objectives are established to eliminate barriers to stable housing.
- Coordinate all necessary support services of the participant. Referrals to other human service agencies for additional services will be made if deemed appropriate.
- Monitor the progress of each participant in relation to their housing plan and needs.
- Responsible for all follow-up activities to achieve the goals.
- Timely completion of all paperwork to assure quick response to the needs of the participant as well as sub-agent needs.
- Maintain accurate records in compliance with agency MDOC and the funding source.
- Function as a liaison with all sub-agents and other community services to assure participants will achieve the goals of their housing plan.
- Provide additional support to Mission Services programs, as needed.
Physical Requirements:
- Sedimentary work, exerting up to 50 pounds of force occasionally and/or up to 25 pounds of lifting. Lifting, carrying, pushing, pulling, bending, reaching, handling, hearing, talking, and seeing.
Working Conditions:
- Inside generally 100%, however, may include outside work on occasion.
- Limited emotional effort. Work environment routine with periods of isolated high stress. Few short deadlines require little or no overtime.
- Travel and the use of a personal vehicle required.
Education:
- Bachelor's degree in human services or related field required.
Experience:
- Two (2) years of experience in housing services preferred. Experience working with returning citizens desired.

- Maintain a good driving record, valid insurance, and valid driver's license.
