About Disability Network Capital Area

Position Location: This position’s main worksite will be at The Davies Project location- 230 Bingham St., Suite 100, Lansing MI 48912. Other locations may include the DNCAP Main Office, remote work environment, and various community locations.

Work Environment: This position involves work assignments in remote working environments such as accessing Zoom and MS Teams web conferencing software.

Minimum Hiring Requirements

Possession of a Master of Social Work from an accredited university.
A minimum of two years’ human service experience with the provision of case management/facilitation.
Personal experience with a disability and/or knowledge of disability related issues.
Must successfully pass a criminal background check, sex offender registry, and DHHS Central Registry.
Must have access to reliable transportation.

Minimum Skill Requirements

Strong verbal and written communication skills.
Ability to design and implement a person-centered approach.
Ability to complete Independent Living assessments. DNCAP assessments may be either office-based or require a home visit.
Excellent organization and documentation skills.
Ability to use word processing and data entry.
Develop and maintain working relationships with community agencies and agency partners.
Must be proficient with the use of web conferencing tools such as MS Teams and Zoom software programs.

Job Description

Information & Referral Specialist/Outreach Services- One Full Time Position- 35-
40 hours per week- In partnership with The Davies Project, Disability Network Capital
Area is seeking a full time Information & Referral Specialist/Outreach Services 35-40
hours per week. This position is a non-exempt position with a starting wage of $18-
$20 per hour. Full benefits package including medical, vacation and retirement. This
position is fifty percent grant funded for one year with a strong possibility of renewal.
The Davies Project is a Lansing based non-profit that provides transportation to
medical appointments for children with long-term and/or serious health challenges,
expecting moms to prenatal appointments, as well as parents to the NICU to bond with
their newborns. The Davies Project is working collaboratively with Delta Dental of
Michigan and Disability Network Capital Area to promote the connection between oral
health and overall physical health. The goal is to help the families remove personal
and systemic barriers by:
1. Education - The promotion of oral health for all family members
2. Goal Setting – Use a person-centered approach to identify, develop, and
implement a person-centered service plan that connects families with other
valuable resources to meet their needs in the community
3. Outreach/Peer Support- Organize and host gatherings with families
4. Create an orientation process for new families of The Davies Project
5. Develop working partnerships with other agencies
6. Complete reports and data entry in a timely manner
The successful candidate for this position will be a Disability Network Capital Area
employee working onsite at The Davies Project. Disability Network Capital Area has a
45+ year history of assisting people to connect with community resources including
healthcare and financial assistance. Supervision for this position includes a fully
licensed Master level social worker and a fully licensed Master level counselor.
Duties include:
1. Providing a range of services either individually or in small groups.
2. Act as a first point of contact for individuals interested in the dental service,
helping to coordinate with local service providers, transportation, etc.
3. Utilize a person-centered approach to develop a plan of service.
4. Assisting individuals with benefits through MI Bridges – food assistance,
Medicaid, and SER
5. Participate in community work groups/committees and develop working
partnerships with other agencies.
6. Timely completion of reports, billings, and data entry
7. Examples of duties outlined do not include all tasks which may be required in this
position. Duties and responsibilities may be added, deleted, or modified at any
time.
