Key Responsibilities
- Provide professional nursing care using the nursing process (assessment, planning, implementation, evaluation).
- Deliver services in clinic, home and community settings, including caring for pregnant individuals and infants under the age of one and their families.
- Provide health education, counseling, and suport to clients and families
- Assist clients in accessing healthcare and community resources
- Collaborate with internal teams, healthcare providers and community partners
- Maintain accurate, timely, and confidential documentation in compliance with HIPAA and agency standards
- Participate in outreach, training, and community health initiatives
- Regular travel within the service area
