Completes Functional Maintenance Programs (FMPs) with the primary goal to maintain resident's highest practical physical, mental, and psychosocial well-being as directed by nursing, therapists, and/or therapy assistants. This includes direct and/or indirect resident care, program planning and implementation, related documentation and communication while adhering to applicable policies and procedures.

PERSONAL QUALIFICATIONS

1. Preference will be given to applicants in the following order of qualifications: 
   1. Associates Degree from a CAPTE/NBCOT accredited program with a valid and current license to practice as a physical or occupational therapy assistant in Michigan.
   1. CNA certified (1 year experience with functional maintenance programs or greater than 3 years experience as CNA preferred) and ability to demonstrate competency of required tasks.

PRINCIPAL DUTIES AND RESPONSIBILITIES

• Implements functional maintenance programs established by nursing and/or rehabilitation therapists to maintain residents' abilities in the following areas: self-care activities of daily living, mobility-related activities of daily living, ambulation, balance, active/passive range of motion, strengthening exercises, turning schedules, bowel and bladder programs, maintaining upper/lower extremity splinting or bracing scheduling, cognitive communicative skills, swallowing exercises, and respiratory exercises.

• Monitors resident's participation and response to programs. Discusses any potential revisions, including discharge of program, with therapy staff prior to implementation. 

• Documents the results of resident's program and any additional requested information in a timely manner.

• Coordinates care with other team members, family, including training and scheduling, with consideration to personal preferences and goals.

• Meets at least monthly with therapy staff to review current programs.

• Attends in-service training and department meetings as required. Assists and participates in facility process improvement programs as required.

• Provides programs in accordance with established standards of practice, department procedures, and efficiency standards.

• Maintains a clean and safe work area by cleaning and disinfecting equipment per facility protocols. Complies with all infection control, universal precautions and OSHA standards for the healthcare professional.

• Ensures appropriate supplies are maintained in the department.

• Assumes responsibility for assignment(s) in the event of a disaster/emergent need to assist the floor staff in the direct care of the resident.

• Must have the ability to work under minimal or direct supervision and maintain composure under pressure within a busy work environment. Must have a positive attitude toward correction and show willingness to continuously improve.

• Maintains positive relationships and rapport with coworkers, residents, family members and other facility personnel.

• Reports to work on time and coordinates schedule to achieve maximum efficiency during assigned shift. Tardiness or absence from the assigned shift must be reported immediately and in a timely manner to the Director or Assistant Director of Rehabilitation. Maintains ability to work varying hours as needed.

• Adheres to established HIPPA regulations.

• Projects a positive and professional image at all times and will always maintain professional communication whether verbal or electronic.

• Assists with resident transfers/lifting and transporting within the building as needed.

• Maintains current licensure and assumes responsibility for ongoing continuing education and professional development to maintain licensure.

• Performs other duties or special projects as assigned.

SUMMARY OF OCCUPATIONAL EXPOSURES

Blood borne Pathogens: Tasks that involve exposure to blood, body fluids or tissue.

All procedures or other job related tasks that involve an inherent potential for mucus membrane or skin contact with blood, body fluids or tissues, or a potential for spills, splashes of them.

PRIMARY PHYSICAL REQUIREMENTS

Physical requirements for this position are classified as medium under the Department of Labor classification. The employee must be capable of:

• Exerting 20 to 50 pounds of force occasionally (less than 1/3 of the time) and/or 10 to 20 pounds of force frequently (1/2 of the time) and/or up to 10 pounds constantly (2/3 or more of the time) to move objects, equipment and/or residents.
• While performing duties of this job, the employee is regularly required to stand, walk, sit, stoop, kneel, bend or crouch, use hands to manipulate tools, equipment, or controls.
• Reach with hands and arms with functional upper extremity range of motion and strength.
• The employee is required to demonstrate typical balance, appropriate ergonomic lifting techniques, and the ability to perform resident transfers that require moderate to maximum assistance with use of the gait-belt.
• The employee is required to have the visual and hearing acuity sufficient to assess client safety and levels of performance.
• The employee works mostly inside client rooms and/or therapy gym.
• This position is classified as Category I for the purpose of exposure determination under the OSHA regulations regarding the Standard of Occupational Exposure to Bloodborne Pathogens (29 CFR 1910.1030).
  • Category I is defined as all procedures or job-related tasks that involve an inherent risk for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills and/or splashes of blood or body fluids. The use of protective equipment and measures is required for every employee engaged in Category I tasks.
