Our private agency is seeking an experienced, dedicated Case Manager to join our youth-centered team. You will play a pivotal role advocating for children, teens, and young adults within Michigan’s child welfare and juvenile justice systems.
Salary Range: $49,920 – $53,760 (based on experience)
?? THE ROLE
As a Case Manager, you will balance compassion with accountability to help young people achieve safety, stability, permanency, and success. You will coordinate care, maintain strong relationships with youth and families, provide or arrange transportation, and ensure case plans comply with MDHHS standards and individual needs.
?? WHAT YOU'LL DO

Caseload Management: Develop, monitor, and update individualized service plans for foster care and juvenile justice-involved youth.

Direct Support & Transportation: Maintain consistent contact with youth, caregivers, and providers. Provide reliable transportation for school, court, medical/mental health appointments, employment, visitation, and community activities.

Resource Connection: Leverage local, county, and statewide resources (housing, education, mentoring, independent living, food/financial assistance) to remove barriers for youth.

Collaboration & Advocacy: Work alongside MDHHS, courts, schools, attorneys, probation officers, and community partners to advocate for youth safety and well-being.

Documentation & Compliance: Conduct required assessments, visits, and staffing. Maintain accurate, timely documentation and data entry in MiSACWIS in accordance with MDHHS policies.

Crisis Response & Relationship Building: Manage emerging safety concerns, maintain professional boundaries, and help youth build transition skills for adulthood.

CWTI Training: Completion of Child Welfare Training Institute training (as required by role).

System Expertise: Strong working knowledge of MDHHS policies, procedures, and child welfare/juvenile justice requirements.

MiSACWIS Proficiency: Skilled in accurate, timely data entry and case documentation.

Hands-On Approach: Demonstrated knowledge of community resources and a willingness to provide direct client transportation.

?? QUALIFICATIONS

Education: Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Criminal Justice, or a related field.

Experience: Background in child welfare, foster care, juvenile justice, or behavioral health.

Certifications & Compliance: CWTI training (as applicable); ability to pass all required background checks.

Requirements: Valid driver’s license, reliable personal vehicle, acceptable driving record, and willingness to transport youth.

Skills: Clear written/verbal communication, organization, and resource navigation.
