TITLE: Registered Nurse, Seasons Adult Day Health Services
No Nights and No Weekends
QUALIFICATIONS:
Education/Training: Registered Nurse. Registration must be current in the State of Michigan.
Experience: Education, hands-on skills and experience working directly with older adults living with
cognitive and/or physical disabilities. Experience and education in dementia and related physical
and psychosocial issues desirable. Must be skilled in working with the family/caregiver and client as
a unit.
Special Skills: Must be professional, articulate, patient, compassionate, and have excellent verbal
and written communication skills. Must possess a strong desire to work with people living with
cognitive impairments. Ability to work effectively as a member of an interdisciplinary team, have
strong leadership, organizational, computer, multi-tasking and critical thinking skills.
JOB GOAL: To support the health, safety, and well-being of adult day participants by providing
professional nursing assessment, medication oversight, health education, timely response to
medical needs in a person-centered, compassionate environment.
REPORTS TO: Seasons Director
PERFORMANCE RESPONSIBILITIES:
1. Assess and manage the medical and health needs of participants upon enrollment and
on a day-to-day basis as required.
2. Oversee all aspects of the processes for administration, dispensing and storage of
medications as prescribed.
3. Routine nursing assessments of health such as weight, vital signs, medications, diet,
cardiac and respiratory systems and others as needed according to regulatory
guidelines.
4. Conduct cognitive and depression screenings.
5. Monitor and provide appropriate intervention for medical emergencies and other health
conditions (e.g. Diabetes).
6. Provide, as needed, nursing services such as nebulizer treatments, skin assessment and
first aid.
7. Communicate with participants, caregivers, and physicians about health related or
behavioral issues.
8. Assist the interdisciplinary team with the development and updates of individual
plans of care.
9. Monitor the infection control program for Adult Day Health Services. Review and
update as needed. Yearly review of Universal Precautions with all staff.
10. Foster personal and professional relationships that may result in new referral sources from
the medical community such as rehabilitation services, physicians, nurse practitioners,
hospital discharge planners.
11. Monthly check of first aid, glucose monitoring and other medical supplies. Obtain supplies
as needed.
12. Health promotion and education to clients, caregivers, and the community as appropriate.
13. Develop and lead health, medical and cognitive-related education and trainings for
Seasons and Senior Services staff meetings.
14. Plan and facilitate monthly Blood Pressure Clinics and Stroke Support Group.
15. Coordinate logistics for Blood Pressure Clinics, including scheduling and communication
with blood pressure volunteers.
16. Other related responsibilities may be required.
