Title: Project Coordinator - Chronic Disease Registry

Purpose:This full-time position (30-40 hours/week) is the recognized resource for development, implementation, and oversight of chronic disease registry projects. The position is responsible for providing programmatic, logistical, and communications support and coordinating registry-related projects that increase chronic disease knowledge and support policy/system change to address chronic disease care cascades. This role will assist in the coordination and implementation for transitioning approved chronic diseases to reportable conditions under the chronic disease reporting rules. Based in the Lifecourse Epidemiology and Genomics Division (LEGD) at the Michigan Department of Health and Human Services (MDHHS) in Lansing, this position works closely with program staff, internal partners including the MDHHS Division of Chronic Disease and Injury Control, and external partners from the Michigan Chronic Disease Registry Advisory Board and other subject matter experts.
Duties and Responsibilities:
Serve as the project coordinator for the chronic disease registries; administer and undertake programmatic, logistical, and communications activities for the project to assure objectives are met.
Coordinate daily operations of program activities in collaboration with appropriate staff, and internal and external partners.
Oversee activities that support provider education, public awareness, and policy/system change to ensure success and timely completion.
Partner with program staff and others to evaluate and disseminate results of project activities.

Support communications by developing and reviewing materials related to the chronic disease registries, such as reports, newsletters, or documentation on data use, governance, disease burden, or education.
Facilitate staff and partner meetings as needed.
Support project management for the chronic disease registry staff to ensure objectives and deadlines are met.
Support expansion of the chronic disease registries through the development, implementation and coordination of processes to review new conditions for potential addition to the state chronic disease mandatory reporting rule.
Develop and maintain materials outlining processes for condition nomination and review. These include but are not limited to infographics, overviews and webpages.
Serve as a liaison between MDHHS staff and the Michigan Chronic Disease Registry Advisory Board to manage inquiries and coordinate chronic disease reporting requirements.
Manage inquiries from members of the public regarding the chronic disease reporting nomination and review process.
Review submitted nomination packages for completeness and gather additional information from submitters as needed to advance review.
Coordinate initial package review with select chronic disease staff members.
Provide routine updates to internal partners and submitters on the status of reviews for nominated conditions.
Maintain the information and materials published to the MDHHS Michigan Chronic Disease Registry Advisory Board webpage.
Manage inquiries from health care organizations and providers regarding reporting requirements and track reporting compliance for reportable chronic conditions.
Coordinate, develop, and implement outreach activities related to chronic disease registry use cases and foster partnerships that further data modernization and use data to address chronic disease care cascades.
Design educational resources, documents, reports, presentations, and display materials; manage communications approval processes.
Strengthen connections with clinical, community, public health, and health information technology partners.
Create an effective communication strategy for disseminating information to partners, data users, and the public.
Manage inquiries from potential data users and ensure alignment with the chronic disease registries' data governance plans.
Refine and monitor the process for managing chronic disease registry data users.
Maintain the chronic disease registry webpages to ensure publicly available information is easily accessible and accurately maintained.
Attend team, section, division, and departmental staff meetings.
Perform other tasks as requested to ensure successful completion of program objectives.
Maintain a positive, strong, credible, professional, and interpersonal relationships with all parties relevant to MPHI projects and represent the best interest of MPHI at all times.

Qualifications/Requirements:
Education: Master's degree; preferably in a biological science, health education, or public health field with knowledge of chronic diseases, public health, health education, or communications.
Experience:No specific experience required, but experience working with federally funded projects and developing health education materials or communication plans is preferred.
ImportantSkillsand Characteristics:
MS Office software skills are essential - Familiarity with spreadsheets, data visualization, and PowerPoint for presentations and posters.
Abilityto maintain records and prepare meeting minutes, grant reporting, and other correspondence.
Experience with health education and outreach methods, social media, and infographics preferred.
