The Registered Nurse (RN) coordinates individual’s physical, emotional and mental health care needs. Works with clinical care managers, supports coordinators, agency practitioners and community providers to coordinate care. Duties may include, but are not limited to medication reviews, health assessments, monitoring, administration, and proper use of medications prescribed. The RN offers health education to staff, providers, consumers, families and the community.
