Advanced Practitioner - Contractual 
(Ex. Physician, Physician Assistant, Nurse Practitioner, or Certified Nurse Mid-Wife) 

District Health Department No. 4 is a dedicated and passionate team that provides leadership in promoting 
environmental and personal health through health promotion, disease detection and prevention, education, and

Duties: 

• Provide direct care and education on pregnancy and family planning methods so clients can make an 
  informed decision.
• Engage with diverse populations, create nursing care plans, and advocate for high-risk populations.
• Provide services in the office setting and/or via tele-health.
• Serve in a manner that is culturally competent and non-judgmental with all clients, coworkers, peers, 
  and supervisors.
• Collaborate with supervision and/or Medical Director as needed to streamline and troubleshoot 
  services.
• Ensure program fidelity to accreditation requirements and nursing standards of care.
• Complete and maintain accurate, adequate, and timely records in computer software products such as 
  Microsoft Office, Electronic Medical Records software, and similar products.
• Utilize effective communication and conflict resolution skills to serve clientele in a tactful, objective, 
  respectful manner while maintaining privacy and HIPAA compliance. 

Requirements: 

• One year of providing community health, pediatric, women’s health and/or family planning services 
  preferred, but not required.
• Valid Michigan identification and reliable transportation.
