Home Health Physical Therapist (PT)

As a Home Health Physical Therapist, you'll provide skilled, one-on-one therapy services in the comfort of patients' homes, helping them regain strength, mobility and independence. You'll assess patient needs, develop individualized treatment plans, deliver evidence-based interventions and educate patients and caregivers to support safe recovery at home.
Whether you're experienced in home health or transitioning from hospital, outpatient, acute care or skilled nursing settings, this role offers the opportunity to practice at the top of your license with greater flexibility, autonomy and meaningful 1:1 patient relationship.

Qualifications:
Current licensure as a Physical Therapist in the State of Michigan
Graduate of an accredited physical therapy program.
At least one year of clinical experience (home care experience preferred).
Valid driver's license and reliable transportation.
Ability to work independently and manage time effectively.
