This position is responsible for the provision of nutrition care, assessment, evaluation, training and monitoring to a patient population (spanning neonates to geriatrics) prior to and following hospital discharge and in ambulatory care areas. Additionally responsible for care plan development and modifications. Care coordination is conducted with prescribers and treatment regimens are established. Orders are entered into an internal software system to ensure accuracy and appropriate provision of all necessary formula, supplies, and equipment to administer therapy. Patients are screened for eligibility with selective payer groups to aid in identifying patient out of pocket expenses. Individuals in this role also triage and resolve problems as they arise with home enteral nutrition therapy, and work as a member of a multidisciplinary patient care team and in collaboration with other health system and home health care professionals. This position will rotate between the hospital and the HomeMed office locations, based on business needs
Essential:
Perform and provide care and service for home care pediatric, adult and geriatric populations.
Evaluate, recommend, and implement comprehensive and specialized nutritional support for patients with complex treatment and diagnostic needs.
Identify enteral feeding access devices and coordinate patient needs related to home enteral nutrition supplies.
Provide training related to home enteral nutrition therapy including enteral feeding pumps and supplies to assigned patients and caregivers.
Monitor enteral feeding regimen and troubleshoot issues related to home equipment and supplies of assigned patient population.
Assist reimbursement staff with insurance issues related to home enteral nutrition patients.
Identify appropriate product replacements when enteral nutrition items are discontinued or unavailable and submit suggestions to the program coordination for review and consideration.
Serve as a consultant to the staff concerning patient treatment plans, established practices, policies and procedures.
Assist in the planning, organization, implementation, and evaluation of student training.
Assign specific cases to students for their study and evaluate the student?s output.
Represent the department at specialty and routine patient conferences.
Assist in developing and conducting special projects or research.
Participate in interdisciplinary planning, for meeting patient care and health care standards.
Evaluate care plans for their appropriateness and initiate necessary changes.
Prepare patients to return to the community by continuing patient education and referrals to appropriate community agencies.
Plan, calculate and record daily dietary intakes of all assigned patients and provide recommendations for therapeutic diets.
Assist with transition from tube feeding to oral diet.
Record treatment provided and patient progress and responses in medical record.

Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
Will comply with all work schedule requirements including shift assignments within any 24-hour period and any day of the week regardless of weekend, holiday, or other calendar recognitions.
Will participate in on-call, 24-hours, 7 days a week, 365 days a year, when assigned.
Will comply with all licensing requirements for working in a pharmacy regardless of the state in which the license is held by HomeMed. For example: Michigan, Ohio, Indiana, Florida, or any other state within the Continental U.S.
