Required Knowledge, Skills and Abilities
- Comprehensive knowledge of Medicare, Medicaid (specifically MI Choice Waiver and/or Community Transitions), and aging/disability resources.
- Experience with person-centered planning principles and addressing barriers to community discharge.
- Demonstrated ability to supervise and motivate staff, mediate conflicts, and solve complex problems.
- Strong oral and written communication skills. Ability to explain complex ideas in a way that is easy to understand and convey the importance of the work. Able to listen to feedback and answer questions from others.
- Competence in Microsoft Office including Outlook, Word and Excel; ability to oversee digital tracking systems (e.g., COMPASS, CHAMPS)
- Maintain strict confidentiality regarding participants, personnel matters, and agency business in accordance with HIPAA and agency policy
- Demonstrated ability to gather information, evaluate it and make decisions based on the information you have.
- Determine the success of TIC programs and make changes for improvement. Evaluate program needs, develop and implement strategies to meet the needs.
- Demonstrated ability to manage multiple projects at once, including creating a project plan, assigning tasks to team members, monitoring the project's progress and evaluating the project's success.
- Demonstrated ability to build and maintain relationships with a wide array of people from diverse backgrounds.
Education and Experience
- Education: Bachelor’s degree in health and human services (e.g. Social Work, Psychology, nursing, etc.), Public Health Administration or related field required.
- Licensure: License in the State of Michigan (LBSW, LMSW, or RN) preferred.
- Experience: Five or more years’ experience in a management position in a social service setting with a focus on long-term care or home and community-based services (HCBS).
- Requirements: Must possess a valid Michigan Driver’s License, current automobile insurance, and reliable transportation.
