Position Summary
The Registered Nurse (RN) works in a collaborative, interdisciplinary environment to deliver excellent patient care through evidence-based practice and individualized care planning. The RN is responsible for facilitating all aspects of the patient visit experience while coordinating care for a defined specialty population and assigned caseload. This role ensures high-quality, cost-effective care across the continuum of healthcare services.
What You Will Need
Education
Graduation from an accredited nursing school and program
Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred
Grand Rapids requirement: If hired with an ADN, enrollment in a BSN program is required within 6 months of hire , with completion within 4 years of hire
Licensure
- Current Registered Nurse (RN) license in the State of Michigan
Experience
Minimum of one (1) year of RN experience required
New graduate nurses must apply through the RN Residency Program , offered three times per year (February, July, and October)
Summary
The RN oversees and coordinates care for patients within a specific specialty area for an assigned caseload. This role facilitates appropriate care delivery, assists patients and families in navigating healthcare services, and supports efficient progression through the healthcare continuum, resulting in high-quality and cost-effective outcomes.
Essential Functions
Completes comprehensive assessments of patients' physical, psychological, social, environmental, financial, and functional status, including review of past and current treatment plans
Collaborates with clinical teams and medical staff on performance improvement initiatives, including assessment, planning, implementation, and evaluation of outcomes for identified patient populations
Serves as a clinical expert for the population served, providing education and consultation to internal staff
Applies critical thinking and problem-solving skills to analyze patient data and develop individualized care management plans
Monitors and adjusts care plans through an interdisciplinary and collaborative approach
Communicates effectively with patients, families, and healthcare team members both verbally and in writing
Participates in the development and implementation of policies, procedures, and quality improvement initiatives for the assigned caseload
Establishes and promotes collaborative relationships with physicians and other members of the healthcare team
Builds collegial relationships with patients, families, and internal/external customers
Acts as a patient and family advocate and facilitates team conferences as appropriate
Provides patient and staff education to promote high-quality care outcomes
Serves as a liaison, negotiator, and facilitator among patients, providers, and third-party payers
Establishes quality goals and metrics aligned with industry standards
Performs other related duties as assigned
