Job Summary:
As the Care Navigation Supervisor, you provide direct supervision of the Care Navigators on the campus of Hickman and Monroe Hospitals.
You will contribute to the development and implementation of policies and procedures to standardize processes and support core competencies. You will oversee the onboarding and integration of Care Navigators into the clinical workflow.
In this role, you will coach and mentor the Care Navigation staff to promote professional growth.

REQUIREMENTS
Education:BSN
Skills:Must have knowledge of regulations and community resources. Must be able to function effectively in a fast-paced environment. Written and verbal skills are essential. Must be able to establish priorities and communicate effectively. History of positive rapport with patients, families, physicians, and interdisciplinary team. Applicable experience in patient education and knowledge of hospital and community resources. Relevant experience in working with diverse populations. Exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in a self-directed environment with attention to detail and follow-through. Computer literate; Microsoft Office competency.
Years of Experience:At least 2 years of previous case management, care navigation or discharge planning experience.
License: Active nursing license in the state in which they are working
PREFERRED REQUIREMENTS
- Education:Master's in Nursing or other relevant healthcare field.
