The Medical Director provides comprehensive clinical leadership and direct patient care for an interdisciplinary PACE program serving adults ages 55 and older. This role oversees medical services for a licensed census of 150 participants (currently 147) and serves as the supervising physician for three Nurse Practitioners. The Medical Director ensures high-quality, patient-centered care, oversees clinical standards, participates in interdisciplinary care planning, and maintains strong professional relationships within the medical community to support participant needs.
This position offers a hybrid schedule, with the ability to work up to two days per week from home, depending on operational needs.
Essential Responsibilities:
Provide direct clinical care within the scope of state licensure, including routine examinations, diagnostic assessments, screenings, and treatment planning for participants 55+.
Serve as the medical leader for the PACE program, ensuring compliance with federal, state, and local requirements.
Supervise, mentor, and oversee three Advanced Practice Providers (NPs), including collaboration and case review as required.
Participate in interdisciplinary team meetings and care coordination for complex geriatric participants.
Maintain strong professional relationships with external healthcare organizations, specialty providers, hospitals, and community services.
Guide clinical protocols, quality initiatives, and standards of practice in alignment with PACE regulatory frameworks.
Provide education to participants and families on health conditions, treatment options, and wellness.
Perform duties consistent with professional standards under licensure and scope, as well as other assigned responsibilities.
Minimum Requirements:
Graduation from an accredited medical school with a Medical Doctorate (MD) or Doctor of Osteopathy (DO).
Licensed to practice medicine in the state of the facility.
Board Certification (BC) required in a primary specialty (Family Medicine or Internal Medicine accepted).
Geriatric Medicine fellowship training or certification strongly preferred.
Must be at least 3 years post-residency.
Experience supervising or collaborating with advanced practitioners preferred.
Ability to work effectively in an interdisciplinary, team-based model.
Current CPR certification (as applicable).
TB questionnaire, PPD, or chest X-ray (per state/facility requirements).
Current health certificate (as required).
Must meet all federal, state, and local regulations and compliance standards.
Must be at least 18 years of age.
