Occupational Therapist (Home Health Private Duty)-PRN/Contract/Per Diem
Position Summary
Join our team as an Occupational Therapist specializing in Home Health Care-Private Duty! In this role, you will deliver personalized therapy to patients in their homes, empowering them to regain independence and improve their quality of life. Your expertise will help patients across diverse settings including geriatrics, and individuals recovering from acute or chronic conditions.
Occupational Therapist will implement personalized therapy plans, promote independence, and support each patient's journey toward optimal health goals through tailored interventions.
Key Responsibilities
- Perform comprehensive patient assessments and evaluations
- Develop, implement, and modify individualized treatment plans based on patient needs and goals. Performs comprehensive occupational therapy evaluations to assess functional status related to activities of daily living and instrumental activities of daily living.
- Provides skilled occupational therapy interventions to improve self-care, cognitive functioning, and adaptive performance.
- Assesses the home environment and recommends modifications and adaptive equipment to promote safety and independence.
- Instruct patients and caregivers in therapeutic, self-care, and adaptive techniques to maximize functional performance and independence.
- Evaluates cognitive, perceptual, and psychosocial factors affecting the patient's ability to perform daily activities.
- Reassesses patient progress at defined intervals and modifies treatment plans based on clinical findings and physician collaboration.
- Coordinates care with nursing, physical therapy, and entire interdisciplinary team members to support patient goals.
- Assesses the need for outpatient or community-based occupational therapy services when services cannot be safely provided in the home setting.
- Foster a positive working relationship with interdisciplinary team members.
Qualifications & Skills
- Strong knowledge of anatomy, physiology, and evidence-based physiotherapy practices across diverse populations.
- Experience in home health, private duty or community-based care is preferred.
- Ability to develop effective, goal-oriented care plans tailored to home health environments.
- Knowledge of geriatric care principles and age-appropriate intervention strategies.
- Experience in orthopedics, geriatrics, sports injuries, and in-home rehabilitation modalities.
- Excellent patient care skills with the ability to motivate, educate, and engage patients in their recovery.
