We are seeking a compassionate HHA/CNA to become a part of our team! You will provide routine in-home care per a care plan and under direct supervision of a nurse.
Responsibilities:
Provide routine individualized care including but not limited to medication reminders, bathing/dressing, meal
preperation, basic wound care, etc.
Monitor and report changes in patient health status
Teach family members ways to care for patients
Transport patients to appointments as well as run errands
Qualifications:
Experience in home health or other health related fields preferred but not required
Positive and patient demeanor
Ability to build rapport with patients
Ability to handle physical workload
Excellent written and verbal communication skills
Ability to re-direct/alleviate patient behaviors
Preferred:
