Job Summary
Responsible for accurate timely completion of the Minimum Data Set (MDS) and Care Area Assessment (CAA), collaborating with Staff Nurses, Certified Nursing Assistants and the interdisciplinary team on the assessment process and development of Resident MDS/Care Plans.
Essential Functions
Establishes weekly MDS schedules and care conference calendars making adjustments as needed.
Ensures timely completion of the CAA by the interdisciplinary team, collaborates with interdisciplinary team on CAA and care planning process, and identifies significant changes in Resident status. Monitors accuracy of MDS items completed by other departments. Performs assessments on Residents for MDS CAA.
Participates in ongoing team conferences and daily clinical round meetings. Monitors Medicare Part A coverage. Informs Social worker of last day of Medicare A coverage. Provides necessary information to the billing department.
Performs audits and quality review duties as assigned by the Administrative Director and or the Director of Nursing. Participates in monitoring and quality assurance committee team meetings. Works with the Director of Nursing to provide staff education and competency evaluations. Provides direct care to the Residents as needed.
Maintains up to date on changing reforms with MDS and PPS systems. Provides PPS information as needed to team members.
Coordinates Resident care plans and communicates changes to the staff. Coordinates discharge teaching with staff. Formulates an individual plan of care, revises plan as indicated by the Resident's response to treatment and evaluates overall plan for effectiveness.
Qualifications
Required
Associate's Degree or equivalent related field
LIC-Registered Nurse (RN) - State of Michigan Upon Hire Or
LIC-License Practical Nursing (LPN) - State of Michigan Upon Hire
CRT-Nurse Assessment Coordinator, Registered (RNAC) - AANAC American Association of Nursing Assessment Coordinators 1 Year required
CRT-Basic Life Support (BLS) - AHA American Heart Association 60 Days Or
CRT-Basic Life Support (BLS) - ARC American Red Cross 60 Days
