Description
Lead Nurse Case Manager
Under general supervision, the Lead Nurse Case Manager delivers community-based health services to individuals and families. This role requires strong clinical judgment, advanced assessment skills, and the ability to interpret and apply public health laws, ordinances, and program policies. The Lead Nurse Case Manager serves as a key communicator, collaborator, and advocate for community health, working closely with internal teams and external partners to address priority health and social issues.
The Lead Nurse Case Manager is responsible for reducing blood lead levels by providing comprehensive, home-based services, conducting environmental assessments, facilitating medical follow-ups, and educating families on lead poisoning prevention. This role also provides clinical consultation to providers regarding reporting and testing requirements.
Examples of Duties
Provide case management services for children with elevated blood lead levels, including follow-up, education, and coordination of care to reduce and prevent further exposure.
Perform capillary blood lead testing at clinics or community events.
Coordinate communication and services with specialty care providers, primary care, other healthcare providers, and Medicaid health plans to address case management and care coordination needs.
Conduct in-home visits to interview caregivers, identify potential sources of lead and evaluate the home environment.
Assess the home environment and overall health of the family to identify risks and needs.
Identify social determinants of health (housing instability, financial stress) that contribute to risk.
Develop individualized plans of care and monitor progress to ensure continuity and effectiveness of services.
Provide education and support to families to promote healthy behaviors and reduce environmental health risks.
Advocate for families to access resources such as lead abatement, legal assistance, or, if necessary, temporary/permanent relocation.
Build and maintain partnerships with community organizations, healthcare providers, and key stakeholders.
Participate in multidisciplinary groups and community initiatives addressing health and social issues.
Identify and integrate cultural, social, and behavioral factors that influence the delivery and effectiveness of public health services.
Strengthen community linkages to improve access to resources and support systems.
Maintain accurate, timely documentation and ensure compliance with reporting requirements.
Contribute to program planning, evaluation, and quality improvement activities.
Interpret, apply, and enforce public health laws, ordinances, and program policies.
Use strong analytical and assessment skills to support program decision-making and service delivery.
Communicate effectively in both written and verbal formats with clients, families, partners, and internal teams.
Performs other related functions as assigned
ADDITIONAL RESPONSIBILITIES:
Completion of HIPAA and FEMA Emergency Preparedness training (toinclude but not limited to:FEMAISCourses: 100,200, 300, 700 and 800) within 30 days of hire.

Minimum Qualifications
Bachelors of Science in Nursing (BSN)
Preferred:
Education,Licensure,Certifications,andExperience
Valid Michigan Nursing License
Knowledge of City Department, Health and Human Services agencies, Qualified Health Centers, and Community Based Organizations at the local and state level.
Documentation in the Electronic Medical Record (EMR) and paper charts.
One year of experience in a hospital or acute care setting as a Registered Nurse(RN)
One year of experience in home care, community health or related field as a Community Health Nurse

This plan is provided by Blue Care Network of Michigan and utilizes their PCP Focus Network. This plan provides you with the opportunity to receive an enhanced level of benefits and pay less out of your paycheck for your medical plan. There are two important requirements that you should consider and understand if you should decide to enroll in this plan. You will have to choose a primary care physician that serves within the PCP Focus network. This primary care physician coordinates your care. The network includes over 22,000 Primary Care Physicians. There are also Wellness Requirements that you (the subscriber) must meet in order to remain in the enhanced level of benefit.
Administrative Special Services Staff (Exempt) are eligible for the following benefits:
CURRENT SICK LEAVE - Granted (3) months of continuous service. (1) day per month, not to exceed (12) days per year.
VACATION - Granted (1) day per pay period, not to exceed 20 days per fiscal year
PAID HOLIDAYS - Granted if paid for 600 straight time hours in the (12) months prior to the holiday
MILEAGE REIMBURSEMENT - Granted on same basis as regular employees.
WORKERS COMPENSATION - Covered by applicable laws.
UNEMPLOYMENT COMPENSATION - Covered by applicable laws.
