Position Overview
Under the supervision of the Patient Care Manager, the Clinic RN assists with Medicare Annual Wellness Visits (AWV’s), providing phone triage, and supporting general nursing functions within the clinic. The RN will work collaboratively with providers and staff to enhance patient care, provide education, and facilitate preventative health services.
Essential Duties and Responsibilities
Conduct Medicare Annual Wellness Visits (AWV), including health risk assessments, screening evaluations, and preventative care planning.
Review and update patient medical histories, including immunizations, medications, and chronic conditions.
Identify gaps in care and coordinate appropriate follow-up services with providers and specialists.
Educate patients on disease prevention, lifestyle modifications, and wellness strategies.
Document all assessments and interventions accurately in the electronic health record (EHR).
Respond to patient phone calls, assessing symptoms, concerns, and care needs.
Provide evidence-based guidance and recommendations based on patient symptoms and medical history.
Determine the urgency of care needs and direct patients to appropriate care settings, including office visits, urgent care, or emergency services.
Offer medication advice, refill assistance, and coordination with providers when necessary.
Document all patient interactions and triage decisions in the EHR.
Review and reconcile medications, provide patient education on new prescriptions, and assist with medication prior authorizations.
Facilitate referrals to specialists, home health services, and community resources.
Provide health education related to disease prevention, self-management, and treatment plans.
Participate in population health initiatives, data tracking, and performance improvement projects.
Work closely with physicians, advanced practice providers, medical assistants, care managers, quality specialists, and administrative staff to ensure comprehensive patient care.
Maintains a cooperative relationship among health care teams by communicating information, responding to requests, and participating in team problem-solving methods

Qualifications

Education and Licensure
Active Registered Nurse (RN) license in Michigan (required). BSN preferred.
Minimum of 5 years of nursing experience, preferably in primary care, outpatient, or a related field (required).
Strong knowledge of Medicare Wellness Visits, preventative care, and chronic disease management (required).
Experience in telephone triage or patient call management (preferred).
Proficiency in electronic health records (EHR) and documentation (required).
Excellent communication, assessment, and critical thinking skills.
Ability to work both independently and as part of a multidisciplinary team.
