Position Purpose
Trinity Health At Home is seeking a dynamic Home Care Account Executive to lead sales and marketing efforts that drive meaningful growth in our home-based care services. In this highly visible role, you will leverage deep community knowledge to build strategic partnerships, expand referral networks, and promote our mission-driven services supporting population health and value-based care.
What You Will Do
Conduct market analysis and collaborate with leadership to develop sales strategies, goals, and account plans.
Execute targeted, solutions-focused sales calls and evaluate effectiveness.
Build and strengthen relationships with referral sources, hospital partners, physicians, and community organizations.
Partner with clinical teams to support discharge planning and ensure smooth patient transitions to home care.
Educate patients, families, and healthcare professionals on Home Care Connect virtual monitoring services.
Screen referrals for accuracy and compliance with state and federal guidelines.
Launch and market new programs and services to priority partners.
Meet or exceed regional admission goals.
Minimum Qualifications
Bachelor's degree or equivalent experience.
At least 2 years of sales or marketing experience; healthcare sales preferred.
Proven track record of achieving sales goals.
Excellent verbal and written communication skills.
Valid driver's license and reliable transportation.
Proficiency in spreadsheets and presentation software.
