Our company is currently seeking a experienced Registered Nurse Case Manager to join our team in providing exceptional patient care. This part-time position is located in Grand Rapids, MI area and will start about 10 hours per week, could lead to full-time. Must have Monday- Friday daytime availability to assist patients. As a Registered Nurse Case Manager, you will play a crucial role in coordinating patient care and ensuring optimal outcomes.
Must be local to the Grand Rapids area and willing to travel locally.

Responsibilities
- Strong utilization management skills to assess patient needs and develop individualized care plans
- Coordinate with interdisciplinary teams to ensure effective inpatient care transitions
- Apply medical terminology and physiology knowledge to enhance patient understanding of treatment plans
- Collaborate with healthcare providers in various specialties including OR, hospital medicine, emergency medicine, primary care, and critical care
- Advocate for patients' needs and facilitate access to necessary resources for comprehensive care
Qualifications
- BSN or Associates and 5+ years of experience
- Certified case manager a plus
- Active Registered Nurse license in Michigan
- Proficiency in utilization management practices and patient care coordination
- Strong understanding of medical terminology and physiology
- Excellent communication skills and ability to work effectively within a multidisciplinary team
- Experience in hospital settings preferred
