Summary The Licensed Practical Nurse (LPN) is aligned under Ambulatory Care Services and will be assigned to various Primary Acute Care Teams (PACT) and/or Sub-Specialty Clinics. The official duty station for this position is at the Lieutenant Colonel Charles S. Kettles VA Medical Center in Ann Arbor, Michigan. Completed work should need only a general review by a registered nurse or physician for appropriateness and conformity with established policies/procedures. Participates in orienting, educating, and training less experienced LPNs/Nursing Assistants (NAs)/or Health Technicians (HTs). Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Provides patient and family education regarding the patient's condition, procedures, and preventive health care reinforces health teaching and promotes healthy lifestyles. Maintains precise adherence to basic principles of asepsis, safety, and universal precautions to provide a safe environment for patients, families, and employees. Participates in training in the setup, use, reprocessing, and maintenance of their clinic/department specific materials/equipment leading to the initial competency and validation of ongoing competency at least annually. Collaborates with interdisciplinary team to support the development, implementation, and evaluation of the patient's plan of care. Utilizes the computer to access patient data to verify appointments, review progress notes and short medication profiles. Orders labs and/or looks up test results under specific clinic profile. Participates and provides input in clinic process improvements and systems redesign activities. Applies knowledge of TJC accreditation standards, Patient Safety Goals, and Infection Control to everyday practice. Promotes a safe therapeutic environment and meets the safety needs of patient, taking into considerations age related needs. Recognizes and reports unsafe working conditions and malfunctioning equipment, initiating corrective measures where indicated. Recognizes life threatening situations and independently performs appropriate care according to approved policies and procedures.
