What You'll Do
- Provide clinical pharmacy services for long-term care residents
- Conduct medication reviews & support optimal therapy
- Collaborate with physicians, nurses, and care teams
- Ensure compliance with pharmacy regulations and LTC standards
- Accurately dispense and verify medications
- Use EHR/pharmacy systems to document care
- Support a positive patient-focused pharmacy workflow
What You Bring
- PharmD from an accredited program
- Long-term care or geriatric pharmacy experience
- Strong clinical skills and attention to detail
- Excellent teamwork, communication, and problem-solving abilities
- Familiarity with pharmacy software (Framework experience is a plus)
