Accurate and efficient documentation of nursing care Knowledge and appropriate use of internal and external resources. This RN position is essential in managing patient care through the pre-transplant stage and may also be involved in chronic disease management. Provide skilled observations, telephone triage, education, and health assessments of patients including history, appropriate interventions, and referrals. Foster continuity of care and facilitate transitions across outpatient and community providers on patient population as needed. Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice. Potential to support outreach efforts.

Knowledge and skills providing patient and family education and reinforcement related to transplant processes, including but not limited to telephone intervention and face-to-face meetings in both individual and group settings. Proficiency with typing and electronic health record documentation. Ability to manage complex clinical issues utilizing assessment skills, protocols, and evidence-based interventions. Willingness to adapt to different populations within Transplant Center after successful Orientation. Orientation and training would be provided for new subspecialties.
Current licensure to practice nursing in the State of Michigan.Michigan Three years of nursing experience as an RN within the last 5 years At least 2 years experience caring for patients with chronic renal disease, general medicine (multisystem care), or relevant experience within the last 5 years Transplant Certification (CCTC/CCTN) within three years of hire.
In order to be considered for this position the applicant must have met or will have met all the required qualifications prior to the start date of employment.
