Accurate and efficient documentation of nursing care Knowledge and appropriate use of internal and external resources. This RN position is essential in managing patient care through the pre-transplant stage and may also be involved in chronic disease management. Provide skilled observations, telephone triage, education, and health assessments of patients including history, appropriate interventions, and referrals. Foster continuity of care and facilitate transitions across outpatient and community providers on patient population as needed. Maintain knowledge of all applicable regulatory laws and requirements, including HIPAA, CMS Conditions of Participation, The Joint Commission, and UNOS to ensure regulatory compliance in nursing practice. Potential to support outreach efforts.

Willingness to adapt to different populations within Transplant Center after successful Orientation. Orientation and training would be provided for new subspecialties.
Current licensure to practice nursing in the State of Michigan.Michigan Three years of nursing experience as an RN within the last 5 years At least 2 years experience caring for patients with chronic renal disease, general medicine (multisystem care), or relevant experience within the last 5 years Transplant Certification (CCTC/CCTN) within three years of hire.

? Experience in caring for kidney transplant patients within the ACU. ? Experience in caring for Liver transplant patients within the ACU. ? Experience with using the United Network Organ Sharing system. ? 2-5 years' experience in the Ambulatory Care setting. ? 1-3 years? ICU experience ? Recent 3 years nursing experience in the inpatient setting ? Knowledge of UMHS nursing policies and standards of practice. ? Bachelor of Science Degree in Nursing.
Salary & Nursing Framework Level: This UMPNC RN posting is posted as Nursing Framework LEVEL C. Actual Nursing Framework LEVEL and salary will be determined at time of hire. Nursing Framework levels range from Level A to Level F.
