The Health and Wellness School Based Facilitator will be responsible for delivering face to face prevention services in schools and the community. The program uses evidence- based models to help at risk youth grow into successful adults. The Facilitator will provide classroom education for adolescents, community education, in service training to community agencies, screening for and facilitation of prevention support groups for youth from high risk environments and participation in community awareness campaigns.
RESPONSIBILITIES:
- Ensure pre/post testing; facilitate substance abuse screenings and linkages primary care, mental health and other supportive services.
- Promote making positive decisions and healthy living through teaching substance abuse prevention and facilitate participant service learning projects (ie. Pledge Drives, Safe Routes & Designated Driver Campaigns) to ensure required program performance goals are met.
- Maintain records and logs of prevention activities, including events, number of persons served and outcome of referrals to primary care, mental health and other support services.
- Work closely with the prevention program participant’s parents to assist them with providing reinforcement of the student’s new skills at home.
- Provide health education to youth, parents and other stakeholders.
- Build and maintain relationships with assigned schools ‘principals, teachers, parent leaders, student leaders and counselors
- Recruit guest speakers/presenters who can educate and motivate youth to make positive attitude shifts toward healthy living and making positive decisions.
QUALIFICATIONS:
- One year of experience working with substance abuse populations.
- Previous teaching experience preferred.
- Ability to maintain rapport with staff, program stakeholders/participants and parents.
- Cultural competency and knowledge of child family dynamics and program planning.
- Must be drug free and available to work evenings and weekends, as needed.
- Must be committed to the success of an evidence-based program.
- Strong classroom management, program planning, management and writing skills.
- Knowledge of communities across Detroit, experience working with diverse populations and at- risk youth.
- Certified Prevention Specialist preferred or must be willing to work toward becoming a certified prevention specialist.
Job Types: Part-time, Contract
Pay: $18.00 - $21.28 per hour
