GENERAL SUMMARY:
Coordinates and facilitates patient admissions and transfers to other facilities within the market, other markets, and other healthcare facilities according to patient acuity, bed availability, and required services.
Responsibilities:
Collaborates with the nursing staff and multidisciplinary team members to place patients in appropriate levels of care to achieve desired outcomes. Coordinates as liaison between physician requestors for consultants and specialty consultants. |
Assists in the care management process including pre-registration and insurance verification. Stays informed of patient changes and communicates the impact to admitting physicians.
Reviews medical reports from referring hospitals or physician offices and relays information to the receiving nursing unit as needed.|
Develops and maintains relationships with referral sources and admission decision makers.|
Maintains standards for documentation and communications with physician/service referrals for legal, medical, statistical, and process improvement purposes.
EDUCATION/EXPERIENCE REQUIRED:
Bachelor of Science in Nursing required.
3-5 (years of RN clinical experience required.
Experience in an acute care setting preferred.
Ability to effectively communicate clinical information and interact effectively with physicians and staff to achieve quality outcomes required.
Demonstrated ability to work in high-stress, high-acuity setting, adaptable to rapidly changing work environment and to direct and control multiple critical communication events simultaneously required.
CERTIFICATIONS/LICENSURES REQUIRED:
- Current license to practice in the State of Michigan as a Registered Nurse
